Certification of Rabies Vaccinations

You must be inoculated against rabies. Please complete and return the Certification of Rabies Vaccination Form by August 1, 2013. This is extremely important as southeastern Pennsylvania is an endemic area of racoon rabies which is beginning to spill over into the dog, cat, horse, and cow population
If you have never been vaccinated (either pre-exposure or post-exposure), you have two choices:
1. Have your series of three rabies vaccinations performed during the summer of 2012 by your health provider.
OR
2. Have them administered to you by the University’s Student Health Services during three evenings in September. The total cost will be approximately $825.00 (3 shots @ $275.00 each). We will make the appointments for you, and sign-up will take place at Orientation. If you hold the health insurance plan offered by the University, these immunizations will be charged to the carrier and will be covered in full. If you do not hold coverage under the University-sponsored health insurance plan, you will be responsible for all charges and billed directly to your student account by Student Health. If your alternate health insurance policy covers these immunizations, you will be responsible for submitting a claim directly to your carrier. After all immunizations have been completed, Student Health Services will provide the Office of Student/Curricular Affairs with a report verifying completion of the series.
If you have already been vaccinated, you must provide us with proof:
1. If your original vaccinations were performed prior to September 1, 2012, you must provide us with a) proof of vaccination from your health provider AND 2) a titer result within the past year indicating adequate immunity. Please attach your proof of vaccinations and your titer with dates of service, and return complete documentation with the Certification of Rabies Vaccination Form.
(If your titer is not adequate, you must also have a booster and include that with your documentation)
2. If your original vaccinations were performed after September 1, 2012 (i.e., within the past year), you must provide us with proof of vaccination from your health provider. You do not need to present a titer. Please attach your proof of vaccinations with dates of service, and return complete documentation with the Certification of Rabies Vaccination Form.
PLEASE SEND ALL DOCUMENTATION REGARDING RABIES VACCINATIONS TO THE OFFICE OF STUDENT AFFAIRS. DO NOT SEND IT TO STUDENT HEALTH SERVICES. DOCUMENTATION MUST BE PROVIDED NO LATER THAN AUGUST 15, 2012, OR YOU WILL NOT BE REGISTERED FOR COURSES.
