Before You Arrive
Please print and fill out the appropriate history form for your pet (please see Reptile, Rabbit and Rodent, Feather Picking, Avian or Avian Flock History forms below). Bringing this form with you to your appointment will allow us to understand your pet's husbandry, diet and routine care in a more efficient manner.
You may bring it with you to your appointment (the preferred method) or fax it to the Special Species Section (fax number 215.573.6050, Attn: Mary Taylor, Special Species Ward) before your appointment. To schedule an appointment, please call 215.898.4680.
